Notice Regarding Students with Disabilities: House Bill 5770, which became law on August 28, 2018, requires that, beginning with the 2019-2020 school year, a school board posts on its internet website and incorporates into its student handbook or newsletter notice that students with disabilities who do not qualify for an IEP may qualify for services under Section 504 of the Rehabilitation Act of 1973 if the child: (i) has a physical or mental impairment that substantially limits one or more major life activities; (ii) has a record of a physical or mental impairment; or (iii) is regarded as having a physical or mental impairment. 

Notice regarding Related Service Logs:  Parents may request a log of related service minutes provided at any point throughout the school year.  Related service providers will provide a copy of the related service logs at IEP meetings and a copy will be maintained in the student's temporary record.

Notice regarding interpreter requirements:  Any parent who is deaf or does not typically communicate using spoken English and who participates in a Section 504 meeting with a representative of a local educational agency shall be entitled to the services of an interpreter. The school will provide an interpreter for IEP, multidisciplinary conferences, and 504 meetings, as well as due process hearings or mediation.

PUNS is a database that registers individuals who want or need Developmental Disability Waiver services (i.e. Community Integrated Living Arrangements, Home Based Supports, Child Group Homes) funded by the Illinois Department of Human Services/Division of Developmental Disabilities. As funding is available, individuals are selected from PUNS and invited to apply for DD Waiver services.  Please contact Jennifer Shaw, the designated trained PUNS process employee, for further information.
